J1 waiver for medical residency training

Hello, I will be graduating from a US medical school next year, currently on a F1 visa. I understand I could either receive a J1 or a H1b visa for residency training.
1) How difficult is it to waive the 2 year home requirement linked to the J1 visa? Is it dependent on area of specialty or your home country?
2)Also is it possible to apply for a green card with a B.A and M.D degree and without going through employment or marriage?
