Please provide the full name of the U.S. citizen who is related to the proposed AOS applicant.

Please select the type of visa the AOS applicant used when he or she last entered the U.S.

In some cases, a foreign national will enter the U.S. in one status (as a visitor for example) and then change status without leaving the U.S. If you have changed your status since you last entered the U.S. please state your current status, the date you changed to your current status and the date that status will expire or did expire.

Please enter the exact date the applicant last entered the U.S. This information can be found on Form I-94 which was issued when the applicant entered the U.S. and is generally stapled to the passport. In some cases, I-94s are not enter but please try to input an accurate date.

If you received an I-94 when you entered the country please input the date that your current status will expire or expired.

For the purposes of this form dependents mean those who were claimed as dependents on the U.S. Citizens most recent federal tax return.

This information is required to determine if the U.S. Citizen petition can meet the financial sponsorship requirements of U.S. immigration law.

For example, if the applicant entered on a tourist visa and was grant 90 days to remain in the U.S. but did not depart the U.S. for 120 days.

Please provide as much detail as possible including specific dates of any time applicant was in the U.S. without authorization.
